The Care Coordination team is at the core of the Healthy Start Program. These dedicated professionals provide personalized support, education, and connections to specialized services for moms and families in our community. Our care is accessible to everyone—no matter their income, insurance status, or residency—ensuring that every family gets the help they need.

Meet the team

Jennifer Luikart

Office Manager

Jennifer is the office manager for the Connect and Healthy Start programs. She has 24 years of
experience working with children and families in various settings. With a particular interest in improving
maternal physical and mental health, birth outcomes, infant health, and developmental outcomes,
Jennifer is a passionate Certified Lactation Counselor who provides free lactation services to eliminate
the barriers to successful breastfeeding. Outside of Healthy Start, Jennifer and her family volunteer in
the community, travel to school events and enjoy downtime at home.​

April Mitchell

Care Coordinator

April is a Care Coordinator in the Healthy Start Program. She is a lifelong resident of
Charlotte County. April has an AA in Early Childhood Development and Psychology. She worked with
children and families as a preschool teacher and director for 25 years before coming to Healthy Start.
April is a Nationally Certified Car Seat Technician, Certified Nurturing Parenting Program Facilitator, and
a certified breastfeeding educator. April is passionate about supporting, educating, and linking her
Healthy Start clients to community resources. When not working in our community, April enjoys her
new adventures with farming and traveling to spend time with her adult children.​

Jordan

Care Specialist

Jordan is a Connect Specialist with over seven years of experience supporting children and families in
Charlotte County. She is passionate about community engagement and helping others, Jordan brings
dedication and compassion to her work. Outside of Connect, she enjoys expressing creativity as a
professional photographer and spending time outdoors with her family going to the beach or kayaking.​

Angie Torres

Bilingual Care Coordinator 

Angie is a bilingual Care Coordinator with the Healthy Start Program. She has over 20 years
of experience working with families and children and is a passionate supporter of pregnant women,
infants, and families. Angie is a Nationally Certified Care Seat Technician and a certified breastfeeding
educator. When she isn’t supporting her Healthy Start clients, Angie enjoys spending time with her
family.​

Megan Trimble

Care Coordinator

​Megan is a Care Coordinator in the Healthy Start Program. She has 20 years of
experience working with children and families. Megan is a certified breastfeeding educator. She has a
background in preschool education and is a strong advocate for early childhood development. In her
free time, Megan has a YouTube Channel where she explores historical sites, camping locations, Disney,
and cruises.

Jesika Palmer

Connect Specialist

Jesika is a Connect Specialist. She is skilled at assessing needs and linking families to
services and resources. Jesika is a Certified nurse assistant and breastfeeding educator with 25 years of
experience. She is a committed advocate for prenatal and family health services. Jesika enjoys spending
time with her family and traveling.
